Use the following steps to register using the Course Registration Form for mail, FAX, or walk-in registration:

- Complete the course registration form.
Call (315) 443-4135 with any questions while completing the form.
Make a careful note of the meeting times and dates for your course selections. Be sure you meet all course requirements and obtain any permissions, if applicable.

Attach or bring your proof of immunization, if not already submitted.
Immunization must be in compliance or your registration will be placed on hold. Submit immunization proof to the Health Center, bring to walk-in registration or submit with mail or FAX registration.
Students taking only online classes are not required to submit proof of immunization.

Enclose or bring payment and other necessary paperwork with your registration.
